The patient was a 20-something year old female with a suspected drug overdose, who arrived by ambulance in status epilepticus. The code blue was called when she suddenly went into an extreme tonic seizure, with possible decerebrate posturing. Her clothes were cut off of her with surgical sheers to give the team better access to her body. She could not be intubated due to seizure activity, so she was bag-mask ventilated on room air. Unfortunately, oxygen saturation could not be measured, because monitor would not stay on patient. A peripheral line had been established en route, so IV atropine was given followed by a 50cc bolus of isotonic crystalloids. No pulse could be palpated and her BP was 60/30, so chest compressions were begun, while waiting for the crash cart.

Once the defibrillator arrived, the paddles were charged and gel applied. A tense struggle between life and death is carried out in a tiny ER suite, with the patient’s anxious partner waiting on the sidelines. Will the scrappy doctors and nurses help the patient pull through, or will death claim another life too soon, leaving only sorrow and heartbreak?
